1- Download the following form  aSimpleDimple Custom Order Form
    If you can't get it to download, let me know and I will e-mail you a copy.      
2- Fill it out
3- E-mail it to aSimpleDimple@gmail.com along with any questions or comments you might have.
4- I'll post your item as "Reserved for (Your-first-name)" under the 'custom order' section of my store.
5- You purchase it and mail me your uniform (or buy one from me).
6- I'll sew your item and ship it to you!
